IVF Treatment: Who Needs It and What It Involves

IVF Treatment: Who Needs It and What It Involves
In vitro fertilisation (IVF) is the most effective assisted reproduction treatment available. It involves stimulating the ovaries to produce multiple eggs, retrieving those eggs in a brief procedure, fertilising them with sperm in a laboratory, and transferring the resulting embryo back into the uterus. IVF is recommended when simpler treatments have not worked or when the clinical situation makes IVF the most appropriate starting point.
Not everyone needs IVF. But for couples with blocked fallopian tubes, severe male factor infertility, significant endometriosis, or diminished ovarian reserve, IVF offers the best realistic chance of pregnancy. For others who have completed several IUI cycles without success, IVF is the logical next step.
At KBR Life Care Hospitals, Sangareddy, we provide thorough pre-IVF counselling so couples understand what is involved, the realistic success rates for their specific situation, the physical demands of the treatment cycle, and the financial commitment. Making an informed decision before starting IVF leads to a more confident, less stressful experience.
Types & Causes
Tubal Factor Infertility
Blocked or damaged tubes mean natural fertilisation is not possible; IVF bypasses the tubes entirely
Severe Male Factor Infertility
Very low or absent sperm count, or poor morphology, requires IVF with ICSI (intracytoplasmic sperm injection)
Advanced Endometriosis
Severe endometriosis damages eggs, tubes, and implantation potential; IVF improves the chances significantly
Diminished Ovarian Reserve
Low AMH or antral follicle count means fewer available eggs; IVF maximises collection of available eggs in one cycle
Failed IUI Cycles
Couples who have not conceived after 3 to 4 IUI cycles have a higher per-cycle success rate with IVF
Unexplained Infertility After Extended Duration
When no cause is found and simpler treatments have not worked, IVF provides the best next-step success rate
Symptoms to Watch For
Infertility not resolved after 3 to 4 IUI cycles
Confirmed bilateral tubal blockage on HSG
Semen analysis showing severely low sperm count or motility
Severely reduced ovarian reserve indicated by low AMH and antral follicle count
Advanced endometriosis affecting tubes or ovaries
Age over 38 with infertility, where time should not be spent on less effective treatments
When to See a Doctor
- After failed IUI cycles or when IUI has been assessed as unlikely to succeed
- When initial infertility evaluation identifies a cause that requires IVF from the outset
- For fertility preservation before cancer treatment (egg or embryo freezing)
- When preimplantation genetic testing (PGT) is required due to a known genetic condition
- Couples using donor eggs, donor sperm, or a gestational carrier
How We Diagnose
- Current ovarian reserve assessment: AMH, antral follicle count, and Day 3 FSH and estradiol
- Updated semen analysis with strict morphology and DNA fragmentation index if not recently done
- Uterine cavity evaluation by hysteroscopy or saline infusion sonography before embryo transfer
- Thyroid function, prolactin, and glucose tolerance check to optimise uterine receptivity
- Karyotyping and genetic carrier testing if indicated by history or repeated IVF failure
Our Treatment Approach
- Controlled ovarian stimulation with daily gonadotrophin injections for 10 to 14 days, monitored by serial ultrasound
- Egg retrieval under sedation: a transvaginal ultrasound-guided needle collects mature eggs
- Laboratory fertilisation: eggs mixed with partner sperm (conventional IVF) or injected individually (ICSI)
- Embryo culture: fertilised eggs develop in the laboratory for 3 to 5 days
- Embryo transfer: one or two embryos placed into the uterus using a soft catheter; remaining embryos frozen if suitable
- Luteal phase support with progesterone; pregnancy blood test 14 days after transfer
