IUI Treatment: The Procedure and What to Expect

Starting IUI treatment raises many practical questions. What injections are involved? How many clinic visits do I need? What happens on the day of insemination? What do I do during the two-week wait? At KBR Life Care Hospitals, Sangareddy, we believe informed patients have a much better experience and are better prepared for the emotional demands of fertility treatment.

IUI treatment follows a structured cycle. It begins with mild ovarian stimulation to encourage the development of one or two good follicles, continues with careful ultrasound monitoring, reaches the insemination step on a precisely timed day, and concludes with a pregnancy test two weeks later. The whole process from start to test takes around 3 to 4 weeks.

The insemination itself is a brief, minimally invasive clinic procedure with no anaesthesia and no recovery time. Most patients return to normal activities the same day. The emotional challenge is often the two-week wait before the pregnancy test, and our team provides support and guidance throughout.

Types & Causes

Natural Cycle IUI

Insemination timed to natural ovulation without stimulation drugs; suitable for women with regular cycles

Stimulated IUI

Mild ovarian stimulation with tablets or low-dose injections to optimise follicle development before insemination

Donor Sperm IUI

Uses screened, frozen donor sperm when the male partner's sperm is absent or severely compromised, or for single women and same-sex female couples

Partner Sperm IUI

Uses fresh or cryopreserved partner sperm processed in the laboratory to concentrate motile sperm for insemination

Symptoms to Watch For

Mild bloating or pelvic fullness during the ovarian stimulation phase

Mild cramping on the day of the insemination procedure, usually resolving within hours

Spotting on the day of or the day after insemination (normal, not a cause for concern)

Progesterone supplementation side effects: breast tenderness or bloating in the luteal phase

Anxiety and emotional tension during the two-week wait before the pregnancy test

When to See a Doctor

  • Severe abdominal pain or bloating during ovarian stimulation (could indicate ovarian hyperstimulation)
  • Heavy bleeding after the insemination procedure
  • Fever or unusual vaginal discharge after the procedure
  • Positive home pregnancy test, to confirm with blood hCG and arrange early scan
  • Negative pregnancy test, to discuss cycle review and next steps

How We Diagnose

  • Baseline ultrasound at the start of the cycle to count antral follicles and check for ovarian cysts
  • Serial follicle tracking ultrasounds (typically 2 to 3 scans per cycle) to monitor follicle growth
  • LH surge monitoring or trigger injection timing to confirm ovulation timing
  • Semen analysis and post-wash total motile sperm count on the day of insemination

Our Treatment Approach

  • Days 2 to 5: oral letrozole or gonadotrophins started after baseline scan confirms readiness
  • Days 10 to 14: follicle tracking scans until a dominant follicle reaches 18 to 20 mm
  • Trigger injection given when follicle is mature, insemination scheduled 36 to 40 hours later
  • Day of insemination: partner produces semen sample, laboratory washes and concentrates sperm, and the doctor performs the insemination procedure using a soft catheter (takes 5 to 10 minutes)
  • Luteal support: vaginal or oral progesterone prescribed for 14 days after insemination
  • Day 28 approximately: blood beta-hCG pregnancy test; positive result confirmed by repeat test and early ultrasound
